Responsible for providing outreach and case management interventions for walk-in clients in support of optimal patient outcomes. CHW - Health Educator - Charge salary range: $19.35 - $22.74 DOE with ability to go higher for highly experienced candidates. 1. Works as part of Family Services Health Education team, in coordination with multiple departments and programs to manage the needs of patients with multiple risk factors and/or barriers to care:
a. Under the supervision of the Director of Programs, CHW will identify client needs and provide needed education and resource connections to the extent allowed by CBHA policy.
b. Assists clients in following through with resource connections, as well as reading mail, making appointments, making phone calls, arranging for transportation, assisting with various applications, etc.
c. Completes and maintains documentation of patient encounters using the Family Services phone note in the electronic health record.
d. Represents CBHA and Health Education team at outreach events.
e. Assists clients by answering questions and teaching how to access needed community services and resources in order to enable clients to be self-sufficient. 
f. Provides referrals in writing or verbal to service providers and community agencies.
g. Provides motivation and support to the patient and family.
h. Develops and maintains relationships with community agencies to optimize resource referral success for clients.
i. Creates and submits encounter reports to supervisor showing productivity no later than Monday at 12:00 p.m. each week.

3. Promotes a positive work culture and upholds clinic policies and procedures, including the WE CARE Standards of Behavior.
4. Must uphold confidentiality at all times in regards to CBHA activities, reports, financials, patient health information and other proprietary information specific to CBHA.
Professional and Technical Knowledge: Position requires level of experience typically obtained through an Associate's Degree in Public Health or Social Services or equivalent education and/or experience. Job duties requires the ability to obtain specific knowledge and training in case management and health education including completion of the DOH Community Health Worker certifcation and the DOH Medical Assistant - Registered licensure, preferably within th first 6 month of employment, but dependent on DOH training date availability.  Licenses and Certification: Possesses a valid driver's license and meet company insurance liability standards as well as the ability to travel to perform home visits and other work related activities.
Technical Skills: Ability to prepare basic correspondence and simple reports in Microsoft Word. Ability to create basic spreadsheets in Microsoft Excel. Ability to create basic presentations in Microsoft PowerPoint. Ability to enter and manage data into a centralized electronic health records and Microsoft Outlook. Communication Skills: Job duties require the employee to effectively communicate using various organizational tools when communicating to supervisor, staff, and patients. Job duties require the compilation and analysis of information prepared in effective written form, including correspondence, reports, articles, or other documentation.  Ability to convince others to take actions in the best interest of the patient and organizational goals. Proficiency in speaking, reading, writing and translating both English and Spanish. -----------------------------------------------------------------------------------------------------------------------
