Community Health Worker - **Seattle Eastside Area** Field-Based position in Seattle Eastside This is a field-based position with daily travel required.
Must be able to meet the required patient visits per day.
Must have reliable transportation and you must be located within the areas of the serviced facilities to be considered for this role.
Valid state driver's license is required.

The Community Health Worker participates in the integrated approach to care management for members. We're looking for a compassionate and motivated individual to join our team as a Community Health Worker!
In this role, you'll play a vital part in helping our members achieve their personal health outcomes. You'll work closely with them to provide support during hospitalization and after discharge, connecting them with resources such as meals, transportation, follow-up care, and nurse case management. You'll also help address social determinants of health barriers, navigate available benefits, and guide them through the healthcare system.

What You Bring To Cambia: Qualifications Preferred: Associate's degree
Clinical certification such as a CNA, Medical Assistant or Community Health Worker Certification.
5 years of health-related experience including at least 2 years of Care Advocate or similar experience or an equivalent combination of education and experience.
Skills and Attributes: Excellent communication and interpersonal skills, with the ability to empathetically engage with diverse individuals on sensitive topics, while maintaining confidentiality.
Strong customer service skills, including active listening, patience, and problem-solving, with a focus on delivering exceptional member experiences.
Proficient in administrative tasks, including data entry, organization, and computer skills (Microsoft Office, Outlook, internet research), with the ability to prioritize multiple tasks and manage workload effectively.
Ability to work independently, prioritize tasks, and meet deadlines, with a proactive approach to taking on additional projects and developing new skills.
Basic knowledge of medical terminology, clinical services programs (e.g., Utilization Management, Care Management, Disease Management), and health-related topics, with the ability to understand clinical documentation and make informed referrals.
Strong collaboration and teamwork skills, with the ability to work effectively with others, contribute to team success, and engage members in their healthcare journey.
Proven ability to motivate and empower others to take action regarding their health, with a creative and solution-focused approach to addressing complex member needs and issues.
What You Will Do at Cambia: Reach out to members through phone or email to engage them in health-related activities and programs, using motivational interviewing and behavioral change principles to encourage participation.
Educate members on preventive screenings, discharge and treatment plans, and provide tools and resources to support their health goals.
Collaborate with Care Managers to identify and refer members to appropriate resources, ensuring their needs are met and connecting them with licensed clinical staff as needed.
Use our care management platform to record and track member interactions, ensuring seamless communication and coordination of care.
Provide support and guidance to members in complex healthcare situations, such as hospital discharge or skilled nursing facility admission.
Empower members to make informed healthcare decisions, navigate care options, and take actions to improve their well-being and prevent/minimize disease.
Identify areas for program improvement and provide recommendations for solutions, supporting the Clinical Services Team with activities such as program communications and training.
