Job Summary: The Community Support Specialist is responsible for clinical services provided as assigned. The Community Support Specialist is accountable for all related clinical, administrative, and quality and compliance responsibilities.
Essential Responsibilities and Metrics: Clinical Responsibilities:
Comprehensive Mental Health Assessments, Development, modification, and review of treatment planCommunity outreachCounselingCrisis interventionCase managementClient centered consultation with community providers or agency staffAttendance and participation at clinical presentationsUse of supervision or consultation when required or needed
Quality and Compliance Responsibilities:
Complete Mental Health Assessment within 3 days of face-to-face assessmentComplete Treatment Plan within 3 days of completion of the Mental Health AssessmentEnsure individual treatment plans are updated and recorded in CIS prior to the six-month due dateModify Treatment Plans as appropriateComplete annual mental health assessments timely and update CISAll Progress Notes will be entered into the medical record as soon as possible or within three business days of the date of serviceProvide timely completion of Level of Care Utilization System (LOCUS) (for adults)Compliance with all Policy and ProceduresMaintain productivity goals as determined by supervisor Administrative Responsibilities:
Keep weekly schedule currentComplete requests time off in a timely manner through supervisorParticipate in Accreditation and Certification processes as directedParticipate in Committee Work as directedParticipate in Strategic Planning as appropriateParticipate in Community Related Services as appropriateRepresent the agency with integrity
