Assist participants with finding community, financial and legal servicesReview and assess caregivers clothing, equipment, supplies and other servicesPerforms any other additional duties that may be requiredParticipates in weekly clinical meetings in the Braintree officeReplacement visits for Vitra RN & CM's on as needed basisConducts weekly calls to AFC Caregivers to confirm compliance with program regulations Education, Experience and Licensure Requirements: At least one years of clinical experience in the care of elders or people with disabilitiesPrevious knowledge of Medical TerminologyAdvanced proficiency in Microsoft Office Suite and Google Suite (G Suite)Valid driver's license with proof of insurance requiredBilingual in English and secondary language preferred
