Role Description This is a full-time hybrid role for a Corporate Licensed Social Worker in Kalamazoo, MI. The Corporate Licensed Social Worker will work to lead LMSW teams across the United States. This will include but not be limited to policy and procedure creation, training, and on-boarding new LMSW in the company. In addition to the corporate role, you will maintain a caseload much lower than that of the large dialysis providers.
It will include providing social work services to patients, including conducting assessments, developing care plans, coordinating resources, and providing ongoing support and education to patients and families. The Corporate Licensed Social Worker will also work collaboratively with the interdisciplinary team to ensure optimal patient outcomes.
