Responsibilities:
-Conduct patient assessments, diagnose conditions, and develop treatment plans in collaboration with the supervising physician.
-Provide direct patient care, including monitoring and managing chronic kidney disease, hypertension, and related conditions.
-Educate patients on their conditions, treatment options, and lifestyle modifications.
-Maintain accurate and timely patient records and documentation.
-Collaborate with the healthcare team to ensure coordinated and comprehensive patient care.
Qualifications:
-Current CRNP licensure in the state of Maryland.
-Board certification as a CRNP in Family or Adult-Gerontology.
-Excellent interpersonal and communication skills.
-Compassion, empathy, and a patient-centered approach to care.
-Ability to work effectively in a collaborative team environment.
