Job Summary:
Under the direction of the Chief of Operations the Day Center Director is responsible for the planning, implementation, and management of the day-to-day operation of the Adult Day Health Center (ADHC).
This includes the supervision of all assigned staff and the provision of services to participants of The PACE Organization of Rhode Island.
Job Duties/Responsibilities: Responsible for the effective operation of the ADHC as the focal point of the PACE model, including meeting the complex care requirements delivered to participants daily.
Involves rigorous communication and coordination with all other departments.
Develop and implement a comprehensive activities program to help meet the physical, social, intellectual, artistic, and spiritual needs of the participants.
Is responsible for non-clinical supervision of all CNAs and dietary aides in the ADHC.
Conducting yearly performance evaluations with the input of the lead CNA(s).
Is responsible for the non-clinical oversight of the medication technician and adherence to medication administration policies in the ADHC center.
Is responsible for all non-clinical supervision of the day center RN and ensuring the quick and timely triage of participants during the day.
Hires, supervises, and trains Life Enrichment (LE) staff so they implement the plan effectively which results in consistent, inspiring, and joyful programming.
Completes annual evaluations.
Supervises maintenance personnel to ensure a clean and safe environment for participants, staff, and visitors.
Oversees contracts for waste disposal and housekeeping services.
Works with the Chief of Organizational Performance to participate in the Quality program to identify quality projects for the ADHC in order to evaluate and improve performance.
Makes emergency arrangements as needed and informs Senior management of all unusual incidents per protocol.
Actively participates in team meetings communicates relevant issues to the team and ensures all center staff are informed and up to date with current information from IDT meetings.
Assists the leads to hire, train and evaluate staff in the center.
Actively engages appropriate performance management protocols and disciplinary action for reporting staff as required.
Ensures staffing is maintained at appropriate levels for safety and recommends staffing levels in planning and budget development.
Ensures compliance with oversight, implementation and adherence to policies, procedures, and standards for all ADHC services.
Collaborates with other PACE Day Center Directors to ensure continuity amongst day center practices.
Leads and serves on various committees of the organization and attends meetings as requested.
Serves as a member of the Operations team.
Serves as a backup facilitator for leading team meetings and/or care planning.
Participates in and contributes to the PACE management team.
Fulfills administrative requirements as requested by supervisor.
Facilitate managing and monitoring participant's progress and potential for decline.
Monitors success of behavioral interventions in the center and reports to IDT.
Ensures participants are oriented to the program and have opportunities to express opinions about their care and services.
Provides excellent customer service.
Conducts and oversees activities as necessary- particularly large events, special gatherings and works with the Marketing team as appropriate Works with the LE Leads to create monthly calendars that details the offerings, ensures choice for participants, responds to the range of clinical needs, and incorporates care plan goals.
Possess strong knowledge of RI communities, interacts with other LE professionals, invites guests and potential partners to get to know PACE and potentially contribute time and talent toward the LE program.
Make recommendations regarding the annual budget to the Chief of Operations to ensure the adherence to financial goals for the department.
Communicates with participants, families, contracted providers, and the public regarding issues related to the routine operation of the ADHC.
Attend and participates in staff meetings, in-services, projects, and committees as assigned.
Provides leadership and direction in IDT meetings for the development and implementation of care plans from an ADHC perspective.
Accept assigned duties in a cooperative manner and perform all other related duties as assigned.
Must be flexible in schedule of hours worked.
Participates in on-call coverage to troubleshoot, advise, teach, coordinate, and participate in service delivery after hours to participants.
Participates in periodic assessments and coordinating 24-hour care delivery.
Serves as member of IDT.
Maintains confidentiality regarding participant, staff, contractor, and organizational information Ability to work effectively and harmoniously with staff, public and private agencies, governmental officials, and the elderly.
Ensures that documentation in team meetings is maintained and changes are recorded in individuals progress notes and/or care plans as required.
Meets regularly with direct reports to review goals, trends, performance measures and data to facilitate change as needed.
Ensures infection control, fire safety, OSHA protocols and other life safety procedures are followed.
In conjunction with Intake and the IDT coordinator/secretary, provides information and responds to inquiries regarding the program.
Assists prospective clients to fullest potential to understand PACE and facilitates follow up communications with intake.
If servicing traditional ADHC clients, serves as a backup for alternate funding sources when applicable.
Completes employee's evaluations in a timely manner on or before their anniversary date.
Creates and implements departmental audit system to identify areas of non-compliance and opportunities for quality improvement; use data to inform decision making and action steps.
Demonstrates an understanding of the importance of clear, thorough documentation as crucial to good participant care; completes and enters accurate, thorough participant documentation in the electronic health record; enters documentation for assessments, orders, and notes within required timeframes.

Education Requirement:
High School Diploma or equivalent, Required
Bachelor's Degree, Preferred
Physical Requirements:
Physical ability to regularly stoop, bend, kneel, lift, stand, walk, stretch and reach for extended periods of time.
Must be able to regularly lift objects up to 25 pounds and at times up to 35 pounds.
Must be able to assist a 200-250 lb participant, using proper body mechanics.
Must have hearing and vision abilities within normal range (corrected) to drive safely and/or to observe and communicate with participants.
Must be able to navigate various departments of the organization's physical premises as well as related community care settings.
Must be able to tolerate conditions typically associated within a medical office and/or home care setting including potential exposure to bloodborne pathogens and infectious diseases.
Must be able to complete assignments in variable weather conditions and ever-changing and sometimes allergenic environments in participant homes, including extreme heat, humidity, dust, smoke, pet contamination, and cleaning supplies.
Experience:
3 to 5 years of related Program Management experience, Preferred
1 to 3 Years of Staff Management Experience, Preferred
1 to 3 years of experience within a health care organization, Preferred
1 year of experience working with a frail or elderly population, Preferred
