Best Point: Education and Behavioral Health is looking for a Full-Time Support and Transition Coordinator to work with students age 8-18 in our Day Treatment (DT) Program on our Madisonville Campus.
The Support and Transition Coordinator is responsible for intake and assessment, coordination between community partners and the DT program, as well as, providing crisis and transition support for clients attending the DT Program.
Assists program manager and staff with implementation of clinical programming, while meeting regulatory standards/requirements.
Primary Responsibilities:
Direct Client Services
Responsible for providing direct mental health service to clients and families within the DT program.
Assists with implementing Individual Service Plans and Behavioral Plans.
Provides clinical crisis support, in the management of crisis situations.
Assists in ensuring that staff is proficient in utilizing de-escalation techniques and interventions.
Provides assistance, in collaboration with care coordinators and DT team, in developing plans for clients who are being transitioned and discharged back to community settings.
Internal and External Collaboration: Collaborates with DT staff, assisting with guardian and caregiver communication.
Attends progress meetings as needed, consults and collaborates with external service providers and resources on behalf of the clients, families and program.
Promotes teamwork and individual accountability, maintaining consistent and constructive communication with all members of the treatment team, internal and external to the agency.
Establishes and maintains positive working relationships with external providers and stakeholders on behalf of the program, department, and agency.
Administration: Responsible for meeting all administrative expectations including but not limited to timely and accurate completion of all clinical documentation and clinical records reporting.
Ensures proper documentation of Incident Reports and Physical Restraint reports, reviewing with staff and guardians as indicated.
Ensures behavior plan documentation is in place for any clients as needed per regulatory standard and agency practice.
Minimum Education, Experience and Other Skill Requirements:
This position requires a Bachelor's Degree in Psychology or Social Work or related field from an accredited college or university plus a minimum of one year clinical experience in working with children and families.
Must possess minimum technical skills including familiarity with Microsoft Windows and ability to use standard Microsoft Office applications.
Work Environment:
Exposed to a combination of office, school and mental health treatment environments.
Exposed to children with behavior problems.
