Department/Location: Community Health Improvement48 hours per pay period Job Summary:Collaborates with members of the health care team to provide medical nutrition therapy.
Identifies patients as high nutrition risk based on criteria outlined by MMH.
Assesses nutrition needs & identifies nutrient requirements according to age & disease specific needs of patients.
Develops & implements the nutrition care plan for all assigned patients.
Fosters continuity of care among all phases across the continuum of care.
Continuously evaluates & monitors effectiveness & outcomes of medical nutrition therapy intervention.
Provides nutrition education to patient & family members on appropriate food selection & eating behaviors to accomplish patient goals.
Actively assesses, evaluates & maintains professional development & competency related to the competencies outlined in the position description.Supervisory Responsibilities:NoneDuties/Responsibilities:Uses 2 patient identifiers before giving treatments, illustrates process.Demonstrates awareness of patient rights.Nutritionally diagnose & assess patients on Med/Surg, Home Care & Oncology according to policy.Develop & implement nutritional care plan for all patients diagnosed & assessed.Monitor & adjust patient's nutrition interventions as needed.Provide individualized nutrition education as needed.Refer patients to necessary support groups, outpatient counseling & classes.Report nutritional care & progress at interdisciplinary rounds.Follow up with physicians, nurses & other disciplines regarding patient care when necessary.Outpatients are contacted & scheduled within one week of referral from physician.Individualized nutrition care plan is developed after thorough nutrition assessment.Patient &/or family educated based on cultural factors, knowledge level & willingness to change.Refers patients to necessary support groups & classes.Accurately documents & charges patient in EHR.Keeps MNT room neat & orderly.
Restocks education materials as needed.Provides follow-up documentation to referring physician as needed.Researches, designs, develops, implements & evaluates educational programs for the community & team members as requested.Coordinates efforts with fellow dietitians.Completes community benefit report & forwards within one week after presentation.Coordinates community nutrition education efforts with other hospital disciplines.Updates &/or order nutrition education materials as needed.Provides class schedule for marketing/advertising to Marketing in a timely manner.Provides inservices to team members & other departments when requested.Attends & participates in department meetings & interdisciplinary meetings.Completes & returns monthly & weekly reports to management in a timely manner.Maintains Professional Development Portfolio per Commission on Dietetic Registration guidelines.Maintains professional competence by attending workshops, trainings, reading journal articles & peer review.Required Skills/Abilities: Strong customer service skills & team player.Effectively communicates both verbal & written.Able to multi-task & prioritize.Able to present to large groups.Able to operate basic office equipment.Education and Experience:Bachelor's degree majoring in Dietetics.Completion of ADA accredited internship or distance learning program.Certified as a Dietitian by the Indiana State Board of Health or licensed by the CDR.No work experience in Dietetics required.Physical Requirements: Frequent walking, talking, sitting, & standing; occasional pushing, pulling, climbing, stooping, reaching, use of hands & manual dexterity.Frequently may need to lift up to 25 lbs.
