Assess nutrition-related needs and goals of patients receiving dialysis using effective communication and listeningskills; collaborate with nephrologists during monthly rounds for assigned in-center hemodialysis patients and monthly clinicvisits for peritoneal and home hemodialysis patients; monitor and document patient progress using electronic medical record.Nutritional Therapy Planning. Provide renal diet education tailored to patients' unique lifestyles and based on patients' renalchemistry results. Develop interventions for patients with abnormal lab results using Bone Disease Protocol and clinicaljudgement. Create nutrition education materials on renal diet guidelines for patients. Communicate with patients, family, teammembers, and dietitians outside facility on appropriate plans of action to ensure optimal nutrition status.Patient Care. Develop plan of care per clinical data, interview, diagnosis and treatment goals.
