We are offering an exciting opportunity to join our esteemed team at Montefiore Wakefield Campus, a Magnet designated hospital recognized for its excellence in patient care.
Our Care Management department is currently seeking a highly skilled Director of Discharge Planning.
The Director of Discharge Planning will report to the AVP of Operations at the Wakefield Campus and be responsible for overseeing and strategizing processes related to patient throughput and discharge.
Key areas of focus include Length of Stay (LOS), discharge volume, high-quality discharge planning, and social work and case management assessments.
The Director will manage a team consisting of the Social Work Manager, Case Managers, and Operations Throughput Managers, ensuring that 24/7 processes are effectively maintained.
A primary aspect of this role will be to establish and maintain external collaborations with skilled nursing facilities, home care, and other post-acute care providers, with the goal of achieving optimal patient experience through process improvements.
Works collaboratively with MHS and Post-acute facilities to ensure safe quality care for our patients, also enhancing the patient/family experience.
In this capacity, the Director will work closely with various hospital departments, including Nursing, the Emergency Department, Inpatient Units, Physicians, and Senior Leadership.
Additionally, the position will ensure compliance with relevant policies and procedures, adhering to legal, regulatory, and accreditation standards.
