Position Overview: We are seeking an experienced Director of Care Management to lead and oversee our care coordination department. This role is critical in managing client care plans, improving client satisfaction, and ensuring the effective operation of our caregiving team. The Director of Care Management will work closely with leadership, field technicians, care coordinators, and caregivers to enhance service delivery and support our growing client base.
Key Responsibilities: Lead the care management team, ensuring timely care plan completion and client satisfaction. Manage, train, and onboard new team members, including care coordinators. Oversee field technicians, monitor spot checks, and ensure home assessments are completed. Develop and implement systems to reduce open tasks and address client concerns promptly. Respond and hands on involvement with care team to resolve escalated issues. Direct communication with clients and responsible parties, including care plan meetings and recommendations to ensure safe services. Collaborate with HR and the hiring team to communicate caregiver staffing needs effectively. Collaborate with Finance to ensure expenses are within budget. Maintain regular one-on-one meetings with team members and manage performance. Qualifications: Proven leadership experience in care coordination or case management, ideally in home care or healthcare. Strong organizational skills with the ability to manage multiple tasks and deadlines. Excellent communication skills, both with clients and within the care team. Experience in training and developing staff members. Familiarity with home care regulations and standards in both Connecticut and Rhode Island is required. Preferred Skills: Ability to implement and manage tracking systems for client care and team performance. Knowledge of home care software systems and scheduling processes (Wellsky Personal Care experience is a plus). Compassionate approach to managing client relationships and resolving complaints.
