Reid & Associates has been engaged to conduct a confidential search for a Director of Care Management for a dynamic healthcare system.


Role Overview: This role oversees the day-to-day operations of inpatient Case/Care Management across multiple facilities across Northern California, ensuring efficient patient flow from admission to discharge.
The Director is responsible for system-wide Case Management, Care Coordination, and Discharge Planning, aligning these functions with strategic objectives.


Key Responsibilities: Oversee and optimize Care Management processes and metrics, including LOS, readmissions, and utilization. Ensure compliance with CMS, CA DCHS, Joint Commission, and other regulatory bodies. Manage inpatient Care Management policies and collaborate with Physician Advisors on audits and reviews. Provide leadership in recruiting, training, and coaching Care Management staff. Coordinate with Revenue Cycle, Utilization Review Committee, and quality leadership to enhance patient care and satisfaction. Qualifications: Master's degree in Nursing, Social Work, or related field. 7-10 years of case management or quality experience, including progressive management in a large healthcare organization. Active CA RN or Clinical Social Worker license, BLS certification required.
