Assists residents with arranging a private caregiver or Home Health/Hospice if resident needs this level of care. Follows up with resident after hospital stay, fall, or illness. Performs treatment of minor wounds. Assists residents in obtaining answers to health- or insurance-related questions. Checks blood pressures for residents as requested. Arranges for flu shots or shingles vaccines for residents. Assists in administering medications such as ear and eye drops according to hospital policies/procedures to ensure resident safety. Keeps accurate and detailed records of resident contacts made. Provides health related educational opportunities for groups of residents. Provides for and follows precautions to encourage resident safety related to fall prevention. Must be able to respond to resident emergency situations. Evaluates hospital- or home-based patients prior to transfer as a special service to hospitals and families. Develops and maintains relationships with outside agencies. Evaluates resident/family needs in relation to resources and refers to appropriate agency or resource. Requirements: Current Nurse license in the state of Colorado, RN Currently certified in CPR and First Aid, long-term care experience preferred Point Click Care experience preferred
