Job Title Family Medicine Medical Doctor (MD) Reports to Chief Clinical Officer Status Exempt (Salaried) JOB PURPOSE As a Family Medicine Doctor at Heartland Community Health Center, you will be responsible for delivering primary care services to patients of all ages, from newborns to seniors. Providers at Heartland possess a holistic approach to patient care, focusing on prevention, health promotion, and the management of chronic and acute conditions. This role involves working in a collaborative, multidisciplinary team to provide patient-centered care that includes Pediatric, Psychiatric, Behavioral Health, and Dental Care.

DUTIES AND RESPONSIBILITIES Conduct comprehensive medical assessments, diagnose and treat acute and chronic illnesses, and provide ongoing care for patients of all ages. Promote health and prevent disease through routine check-ups, screenings, immunizations, and patient education. Maintain accurate and timely patient records in compliance with medical and legal standards. Manage chronic conditions such as diabetes, hypertension, anxiety, depression, and asthma, developing personalized care plans to ensure effective treatment and monitoring. Educate patients and their families on health conditions, treatment options, and preventive measures to promote long-term wellness. Collaborate with other healthcare providers, including specialists, nurses, social workers, and behavioral health professionals, to ensure comprehensive and coordinated care. Participate in community outreach programs, health fairs, and educational events to promote health awareness and preventive care. Participate in community-based care coordination and partner referral agreements for specialized care. Contribute to quality improvement initiatives within the health center to enhance patient care and outcomes. Advocate for patients' health needs, addressing social determinants of health and promoting health equity within the community. Participate in on-call coverage as needed (typically one week/month). Participate in value-based care and quality assurance programs. Provide leadership within the clinical team, guiding best practices, participating in quality improvement efforts, and mentoring other healthcare staff. Adhere to the Health Insurance Portability and Accountability Act (HIPAA) regulations by protecting patients' rights and maintaining confidentiality of personal and financial information. Maintain certifications and licensure through continuing education requirements as specified by the certifying bodies. QUALIFICATIONS Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited institution. Completion of an accredited residency program in Family Medicine. Board certified or board eligible in Family Medicine, or the American Osteopathic Board of Family Physicians. Valid medical license to practice in Kansas. Valid DEA license. Valid Basic Life Support Certification. Valid Mental Health First Aid Certification (completed upon hire). Previous experience in a community health setting is preferred but not required. PHYSICAL REQUIREMENTS Moves equipment weighing up to 25 pounds. Moves throughout the building to meet with employees, patients, or visitors. Must be able to remain in a stationary position during shift. Ability to read, write, and speak English. Frequent communication with patients about their experience at the health center. Must be able to exchange accurate information in these situations. Operate a computer and other office equipment, such as tablets, scanners, printers, and phones. Adhere to process protocol or procedures. DETAILS Schedule: Full-time non-exempt (hourly). Hours Mon-Friday 8:00AM - 7:00PM (as needed) (average 32 clinic hours/week) Shifts are typically 8 hours. Location: Heartland Community Health Center at River City (1312 W.6th Street)
