Family Medicine Physician

Provides medical care to family medicine/Internal medicine patients, regardless of age or sex. Diagnoses illness and injury. Consults with patient to outline treatment and prevention plans. Maintains records and charts according to laws and regulations. In addition, the physician is part of a Patient Centered Medical Home (PCMH) team and is expected to be familiar with the philosophy, goals and objectives of PCMH.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.May manage the daily operations of a specific medical program, patient care unit, or research function.Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.As appropriate for the position, participates in specified health promotion, education and/or prevention programs.Supports and complies with the Principles of Practice including the vision and mission of Family Circle of Care.Exhibits behavior consistent with the Clinic Partner Profile and Physician Covenant.Reviews patient file/record, including allergies, problems, medications, and immunization status.Elicits and records information about patient's medical, developmental, social, and family history.Examines patient to determine general physical condition or presence of disease or injury.Orders or executes various tests, analyses, and diagnostic images to provide information on patient's condition.Analyzes reports and findings of tests and examination, and diagnoses condition of patient.Administers or prescribes treatments as needed.Determines and prescribes medication, dosage, and schedule given the patient's condition and allergies.Prescribes and may administer vaccinations to immunize patient from communicable diseases.Discusses any possible side effects to medication with patient.Refers patients to medical specialist for consultant services when necessary for patient's well-being and further medical evaluation and/or interpretation.Follows up with patient regarding progress in high risk or emergency cases.Must code visits correctly for payment to clinic.Documents the patient's visit including medical history, physical exam, diagnoses, and plan of action.Participates in CPI program. Maintains compliance, is knowledgeable, and utilizes the guidelines of all state and federal laws and regulations as they pertain to the position including, but not limited to; Health Insurance Portability Accountability Act (HIPAA), Scope of Practice, Accreditation Standards, Occupational Safety and Health Administration (OSHA), Bureau of Primary Health Care/ Health Resources and Services Administration (BPHC/HRSA), National Committee for Quality Assurance/Patient Centered Medical Home (NCQA-PCMH), Centers for Medicare and Medicaid Services (CMS), Clinical Laboratory Improvement Amendment (CLIA), Joint Commission on Accreditation of Healthcare Organizations (JCAHO), and other HRSA related regulatory functions and/or agencies that Family Circle of Care participates in.Patient Centered Responsibilities Provides and manages direct patient care in a clinical team based approach centered on patient needs and incorporating individual patient preferences in the delivery and approach of care, including physical examinations, evaluations, assessments, diagnoses and surgical treatment for a specified patient population.Ensure care coordination, referral management, clinical workflow processes by meeting regularly with clinical staff.Provides guidance to patient and family members when applicable regarding nutrition, safety, discipline, injury, and disease prevention and any other educational materials.Supports and complies with the Family Circle of Care principles of practice and patient centered care model including the mission and vision.Knowledge, Skills, and Abilities Required This position requires that the individual demonstrate the following:
Ability to observe, assess, and record symptoms, reactions, and progress.Knowledge of legal and ethical standards for the delivery of medical care.Knowledge of relevant drugs and non-pharmaceutical patient care aids and ability to prescribe dosages and instruct patients in correct usage.Ability to maintain quality, safety, and/or infection control standards.Ability to develop and present educational programs and/or workshops.Knowledge of related accreditation and certification requirements.Knowledge of community medical diagnostic and patient care services in area of medical expertise.Knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment in area of expertise.Effective verbal and written communication skills.Ability to supervise, advise, and train clinical professionals and/or students in area of expertise.Ability to work both independently and in a team environment.Supervisory Responsibilities Assists and supports Site Manager in the training and supervision of clinical medical support staff, mid-level practitioners and students. Carries out training and supervisory responsibilities in accordance with the Family Circle of Care policies and applicable laws. Directs and coordinates the clinical patient care activities of nursing and support staff as required. Responsibilities include planning, assigning, and directing work flow; addressing complaints and resolving problems.

Education and/or Experience Doctoral degree from an accredited school of medicine required, completion of postgraduate residency training in Family Medicine. 3-5 years' experience preferred but not required.
Equipment Working knowledge of clinical equipment i.e. scales, thermometers, dynamap, nebulizers, glucose meters, pressure monitors, and any other point of care testing equipment.
Computer Skills To perform this job successfully, an individual should have knowledge of Electronic Medical Records systems, EPIC preferred, Microsoft Excel Spreadsheet software and Microsoft Word Processing software.
Certificates, Licenses, Registrations Unrestricted licensure to practice medicine in the state of Texas; BLS, and PALS Certified; Federal DEA Certification; Board certified or Board Eligible in Family Medicine. Must be credentialed per Christus Trinity Mother Frances Health System policies and procedures; employee responsible for all renewals and CME requirements needed for all licensure and credentialing.
