Position Overview
The Family Team Facilitator plays a vital role in supporting youth and families involved in the Juvenile Justice system. Using a wraparound approach, the Facilitator collaborates with families, youth, and their support networks to identify needs and develop personalized care plans.
Key Responsibilities Facilitate meetings with youth, families, and support networks to create care plans.Conduct strength-based assessments to identify needs and resources across all life areas.Build strong relationships with youth, families, caseworkers, and support systems using effective interpersonal skills.Deliver care that is strength-based, family-centered, and culturally responsive.Track service progress and maintain accurate electronic data for quality assurance.Lead family team meetings, complete assessments, and advocate for youth and their families. Qualifications Education/Experience:Bachelor's degree OR 3–4 years of experience working with high-risk youth in the Juvenile Justice system.Skills and Training:YASI (Youth Assessment Screening Instrument) training preferred.At least 2 years of experience coordinating care and working directly with youth and families.Strong interpersonal and communication skills.Ability to work flexible hours, including evenings and weekends, to accommodate families' schedules.Exceptional attention to detail in service coordination.
