About the role: Our Float Advanced Practice Clinicians are a key part of our core care model supporting  our mission to provide complete dignified care to every patient we serve by providing coverage for temporary staffing gaps and high patient volumes. 
Each of our patients are followed by a longitudinal care pod which includes a team consisting of an Advanced Practice Clinician, Community Health Workers, and Patient Experience navigators who care for patients in the home and community. This team is responsible for providing and coordinating holistic, patient-centered care with complex medical, behavioral health, and social needs to improve health outcomes, wellbeing, and maximize healthy days at home. Our care pods are supported by Physicians, Social Workers, RNs, Behavioral Health Clinicians, Psychiatrists, and Pharmacists who are pulled in as needed.  
The Float APC position is a hybrid role that does not carry their own patient panel but rather supports our longitudinal care pods by providing clinical coverage for pod APCs when needed to support our medically complex patient population.  This will include delivering excellent in-home and virtual care for initial visits, follow up visits, and occasionally urgent care visits. The Float APC will work closely with our longitudinal care pods to ensure appropriate clinical management and handoff of patients.

\n Responsibilities will include:Anticipate 70-75% travel, which will change frequently based on staffing needs of the primary teams.  This role will cover all of our geographies within the greater Detroit Metro Area (Wayne, Oakland and Macomb counties).  Clinical Coverage and Panel Management for OOO ProvidersProvide coverage of planned initial and follow up visits (both in person and virtual)Clinical follow-up for labs and/or studies ordered during visits conducted, determining appropriate care plan, communicating with the patient, and ensuring closed-loop follow-up and handoff to appropriate longitudinal team membersSupport urgent clinical tasks for OOO provider panel, including refill management and result follow-upsEstablish and foster trusting relationships with your patients and ensuring that care is appropriately aligned with their goals and valuesClear and accurate clinical documentation to support the primary team understanding next steps of the clinical planCollaboration with a multidisciplinary team including RNs, Community Health Workers, Behavioral Health Clinicians and others   In-home urgent care visits for Accompany Health patients in order to help evaluate and address urgent clinical needs and maximize patient healthy days at home Other Supporting Tasks as neededVirtual Clinical Triage and Urgent Care supportPRN inbox support for providers who are in the field Preventative care support for quality campaigns, including patient outreach, education, and appropriate evaluation / assessmentRegular participation in weekend and off-hours on call schedule for urgent clinical escalations Provide feedback on program design and workflows to ensure we are providing the best patient care possibleEnsure timely and appropriate documentationRoles and responsibilities may evolve as our care model develops including virtual coverage of additional markets What makes you a fit for the team:Passionate about caring for complex, historically underserved patients with co-occurring chronic and behavioral health conditions in an integrated, multi-disciplinary model anchored in home-based and tech-enabled virtual care.Committed to providing the highest quality, outstanding clinical care to all patients, regardless of their needs.Ability to be flexible and adapt to schedule changes that best support our patients and market Strong proficiency in using technology and delivering a high caliber of care virtually.Composure under unpredictable circumstances as well as strong clinical assessment and critical thinking skillsExcellent communicator, collaborator and team player who enjoys working in an integrated, multi-disciplinary model.Committed to providing the highest quality, outstanding clinical care and willing to go the extra mile for all patients.Possess high attention to detail as well as adaptability, and is excited to be a part of and contribute to the development of a rapidly evolving, innovative care model.Enjoys continuously learning and adapting workflows to improve patient care. Desired skills and experience:RequiredActive Physician Assistant or Nurse Practitioner license in home state and willingness and certification in good standingAANC or AANP board certification for Nurse Practitioners OR NCCPA board certification for Physician AssistantsActive DEA Controlled Substance Registration or eligible for application. X-waiver licensure (for buprenorphine prescribing) is a plus but not requiredActive controlled substance license in state in which you are seeking employment or eligible for applicationCertified in Basic Life Support for Healthcare Providers3+ years of clinical experience in providing primary care in an internal medicine, family medicine, geriatric medicine or urgent care settingComfort and experience in working within an electronic medical recordPossess exceptional clinical triage, and clinical assessment and urgent care skillsDemonstrated ability to help a patient adapt new habits, change behaviors, and motivate towards achieving health goalsComfort with electronic medical record documentation and excited about how technology can support your work and drive ongoing improvement towards new and better careExperience and comfort working within an interdisciplinary care team, and specifically communicating with clinical and non-clinical team membersValid unrestricted driver's license and access to an insured vehicle for daily use Preferred Experience providing care in home settingExperience in clinical triage and urgent careExperience in value-based care organizations
