Job Summary:
In accordance with agency Mission, and in using Trauma Informed best practices, the Housing Retention CSS provides additional housing and community support services to clients enrolled in CPR who require assistance in obtaining and retaining affordable and safe permanent housing.
The Housing Retention Specialist works collaboratively with team members and agency departments in identifying individuals experiencing homelessness or who are at imminent risk of homelessness and eligible for PATH enrollment.

Essential Job Functions:
+ Responsible for providing community support services related to housing retention.
This includes working with clients on completing housing stability plans; completing housing applications; explore emergency housing resources to prevent homelessness; and assisting clients with completing one-time emergency assistance applications when needed.

+ Works with clients receiving rental assistance through Supportive Community Living (SCL) Voucher in finding and securing other rental assistance programs and housing options.

+ Assist clients primary case manager as needed in providing supportive services which promote housing retention, community integration, life skills, and improved quality of life.

+ Identifies individuals who are homeless or at risk of becoming homeless and PATH eligible.
Ensures that PATH enrollment documentation and data is entered into Caseworthy in an accurate/timely manner to ensure individuals are tracked within this system and linked to appropriate housing supports.

+ Attends all related PATH trainings and community wide meetings such as coordinated entry meetings through the Greater Kansas City Homeless Coalition.

+ Responsible for assisting eligible PATH identified clients to complete Vulnerability Assessment Tool (VAT) and doing so in a timely manner.

+ Maintains good working knowledge and up to date list of community resources that would benefit those consumers dealing with housing issues and homelessness.

+ Ensures all agency departments are educated and knowledgeable regarding PATH funding and are aware of how to refer those clients that are appropriate to receive funding and services.

+ Knowledgeable of all agency program criteria for successful admissions into the program.
Collaborates with the appropriate staff/programs or other community agencies to collaborate continuity of care.

+ Completes all documentation as defined by program, agency and DMH/Medicaid standards.
Ensures that all documentation is entered into the electronic medical record in a timely fashion according to agency policy.

+ Serves as a vital multi-disciplinary team member, and facilitates program, departmental and agency communication/operations and shares information with team to develop more comprehensive services for the program.

+ Follows all agency policies and procedures including emergency and corporate compliance policies/procedures 100% of the time.

+ Provides case management and crisis intervention for clients as requested by supervisor.

+ Serves on departmental, program, or agency committee(s) or task team(s) as assigned.

Experience and Education Qualifications:
+ Bachelor's degree in psychology, social work, nursing, counseling or closely related human services field.
If the degree is in a field other than human services, a bachelor's degree and four years of experience in mental health is required.
Equivalent experience may be substituted on the basis of one (1) year of experience in case management for each year of required educational training.

+ Preferred experience in working with the homeless population is preferred.

+ Licenses, Certifications, or Registrations: Training or certificates on SOAR, PATH and Caseworthy is required, but will be provided if not obtained prior to hire.

Supervisory Requirements:
+ None.

Employment Requirements:
+ Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check.

+ Completion of New Hire Orientation at the beginning of employment.

+ All training requirements including Relias at the beginning of employment and annually thereafter.

+ Current drivers license, acceptable driving record and current auto insurance.

+ Must acquire and maintain required training per agency policy and DMH regulations to include documented attendance of professional development/training or education.

+ Must have access to dependable automobile with adequate auto insurance and registration, for making frequent home/community visits.

Physical Requirements:ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up the 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body.
Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time.
Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
