This role is critical in developing individualized treatment plans, coordinating comprehensive services, and ensuring that foster children receive the medical, social, educational, and other services they need. Develop and implement individualized treatment plans for children in foster care.Prepare and maintain detailed case records and complete weekly activities/status reports.Coordinate and link children to appropriate services to minimize care fragmentation and ensure continuous access to necessary services.Facilitate relationships and interactions between birth parents, foster families, and children to support family success and child welfare.Monitor and continually evaluate each child's care plan, adjusting strategies as necessary to meet evolving needs.Assist in the recruitment, assessment, and training of foster parents.Engage in collaborative relationships with foster parents to help them provide a safe, nurturing environment for foster children.Coordinate services and service planning with other agencies involved with the child by arranging medical, remedial, and dental services as needed.Collaborate closely with family assessment and planning teams to prepare all case plans and coordinate services.Ensure all documentation is completed and submitted within established timeframes in accordance with company policy.Maintain compliance with Health Connect America (HCA), State, and Council on Accreditation (COA) health and safety requirements.Resolve stakeholder issues promptly and proactively to avoid complications.Maintain high-quality work standards, focusing on continuous improvement and quality.Communicate effectively with all stakeholders, keeping them informed and updated.Collaborate effectively within the team, accepting feedback positively and contributing to team efficiency.
