We are looking for a Full-time Nurse Practitioner (NP) for a Hospice position.
The Nurse Practitioner would work in the field and inpatient at the hospice house.
The Nurse Practitioner must haves some hospice experience.
The Hospice Nurse Practitioner serves as a member of the interdisciplinary team with the Medical Director, Clinical Managers, Social Worker, Pastoral Care Coordinator, Volunteer Coordinator, Primary Nurse and others, confirming that the patient meets the medical criteria for admission to the hospice program based on available prognostic indicators.
Successful outcomes of this position include high quality patient care provided in a compassionate, competent, and caring manner; clinical program development; skill development for Hospice clinical staff, regulatory compliance with proper documentation enhanced community relationships, program visibility; excellent staff and patient satisfaction.

REQUIREMENTS FOR THE HOSPICE NURSE PRACTITIONER POSITION: The Nurse Practitioner must haves some hospice experience.
