About the role: As a Health Advocate at Accompany Health you will serve a critical role on our integrated care team. Our Health Advocates are responsible for building trust with our patients and empowering them to live healthy, dignified, independent lives. They are "natural helpers" who are thoughtful and creative in figuring out how to support people in their community.
\n Responsibilities will include:Building trusting, longitudinal relationships with our patients through in-home, virtual, and community-based visitsCollaborating with our clinicians by joining their medical visits and assisting with vital collection and screeningsConducting an initial assessment for all patients to identify their goals and needsConnecting patients to local resources and social services, for example helping them apply to SNAP benefits, accompanying them to a senior center, or researching community support groupsSupporting patients with chronic disease to take charge of their health, manage their medications, and stay out of the hospitalHelping patients navigate the healthcare system by accompanying them to doctor's appointments, visiting them when they're in the hospital, and helping coordinate their appointmentsProviding health coaching to help patients with behavior change like exercise, smoking cessation, healthy eating, or helping them complete preventive care screeningsWorking with your team to build and keep up to date a resource directory of local and national resources (i.e., transportation, housing, financial, food, medication discounts, support groups)Documenting all care and patient communication in our electronic medical recordAttend regular meetings  What makes you a fit for the team:Passionate about caring for people in your communityTeam player mentality with a can-do attitudeHighly organized and comfortable multi-taskingSkilled at  advocating for your patients to internal and external teamsCultural competency and enthusiasm for working with diverse groups Desired skills and experience:Required:Resident of the Detroit Metro area with an understanding of local resources and experience working with patients to access themExperience providing home visits and/or telehealthExperience working with medically complex, elderly, or frail populationsYou are comfortable working with members in-person in their homes 4 days a week and also over video and phoneYou understand that the role is a hybrid role split between 4 days per week in home visits and 1 day in the officeFlexibility to work Monday-Friday 8:00am-5:00pm with willingness to work 10:00am-7:00pm or 11 am-8:00 pm ~2 days per month. Additional flexibility to work an occasional evening or weekend day. Please note that the schedule may be adjusted to accommodate patient care and operational needs.Your attributable geography (subject to change) can cover an area that will require variable drive times Must have a valid unrestricted driver's license and access to an insured vehicle for daily use as driving is an essential function of the role PreferredExperience measuring various physiological functions to assess a patient's overall health (patient vitals and conducting screenings)Community Health Worker Certification or an Associates degree or higher in health sciences or related fieldExperience as a Health Coach3+ years' work experience in Community Health Worker or case management roleExperience navigating hospitals and health insurancesProficiency with electronic medical records or other documentation software

For Patient Facing Roles To keep our patients, communities and each other safe, you'll be required to comply with Accompany Health's medical clearance requirements, including completing a TB screen and providing proof of immunity or vaccination for certain conditions. You will also be required to be vaccinated and up-to-date on your COVID-19 vaccinations, including boosters. This is a condition of employment, and we make exceptions as required by law.
