We are conducting a search for our client, an FQHC, federally funded health community health center, and not for profit organization. They are seeking a full-time (35-40+ hours a week) Physician (Family Medicine Physician) for the role of Health Center Medical Director (HCMD) at their downtown Chicago location.
Calling all Family Medicine Physicians (MD or DO). English/Spanish or English/Polish is preferred.

You will be the primary practitioner, onsite, at a school-based program, which is also seeing patients from the community, at this location. The program was developed to be a primary resource for communities that are often impacted by health inequities.
Some of the needs can include chronic disease management, adolescent pregnancy, mental health support (with additional guidance from an on-site team member). Requirements: 5 years of Provider/ Physician experience, minimum.Board certification in specialty must be kept current, in Family Medicine.Unrestricted, active license to practice as a Physician (MD or DO).Active, current Controlled Substance license within the State of Illinois and federal DEA certificate, without restriction.Up-to-date provider-level BLS CPR.Prior experience in a community health center and/or federally qualified health center, or a school setting.Mental health expertise, a plus. Patient Template The Center Medical Director is scheduled to see patients 8 sessions per week. Health Center Medical Director has HCMD Administration time 1 session per week. Health Center Medical Director has Clinical Administration time 1 session per week. Providers are scheduled for one evening per week. Hours of coverage on this day are: 11:30am to 8pm.
Call Schedule All providers in the entire practice rotate covering after hours off-site call. Monday – Friday. 5pm to 8pm. Residents cover call Saturday and Sunday for the entire weekend. All providers receive a $50.00 monthly telephone stipend.

Base Compensation, based on years of experience: $163,200 to $213,180 annually.
