Navajo Preference Employment Act

In accordance with Navajo Nation and federal law, TCRHCC has implemented an Affirmative Action Plan pursuant to the Navajo Preference in Employment Act. Pursuant to this Plan and corresponding TCRHCC Policy, applicants who meet the necessary qualifications for this position and (1) are enrolled members of the Navajo Nation, Hopi Tribe, or San Juan Southern Paiute Tribe will be given preference in hiring and employment for this position, (2) are legally married to enrolled members of the Navajo Nation, Hopi Tribe, or San Juan Southern Paiute Tribe and meet residency requirements will be given secondary preference, and (3) are enrolled members of other federally-recognized American Indian Tribes will be given tertiary preference.


POSITION SUMMARY Position is responsible for the delivery of evidence-based curricula to at-risk youth and adults participating in the childhood obesity intervention program, Take C.H.A.R.G.E.; and diabetes prevention program, Lifestyle Balance. Position will provide coaching and support to participants throughout the intervention program utilizing evidence-based strategies. Work duties include the use of health-related assessments and tools to develop health improvement plans for at-risk youth and/or adults. Incumbent will assist the Diabetes Prevention Coordinator in the monitoring and evaluation of health outcomes of participants; and other activities that support the SDPI grant goals and objectives. Understanding and experience with electronic health records. Knowledge of local tribal philosophy and cultural practices is a must. Incumbent must be willing to work evenings and weekends, as needed. This position is grant funded. NECESSARY QUALIFICATIONS Education: Bachelor's degree in Exercise Science, Physical Education, Kinesiology, Nutrition, or similar health-related field
Experience: One (1) year demonstrated experience in the area of health education or coaching principles, practices, methods, implementation, and evaluation of preventive health service programs.Must have one (1) year experience in diabetes prevention programs for at-risk youth or adults or related health programs.Demonstrated experience in behavior change, motivational interviewing, basic nutrition and fitness principles (extra training and certifications of trainings will be required within six (6) months of hire).Licensure/Certification: Must have and maintain a Basic Life Support Certification (BLS) from the American Heart Association (AHA) or must obtain within six (6) months of hire.Training or Certification in Group Fitness or Personal Fitness or must obtain within six (6) months of hire.Must maintain and have an unrestricted current valid state driver's license.Other Skills and Abilities: A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers.Positive working relationships with others.Possession of high ethical standards and no history of complaints.Reliable and dependable; reports to work as scheduled without excessive absences.Basic knowledge of data collection, analysis, practice, and ethical considerations.Proficient with the following software programs: Excel, Word, Power Point, Outlook, Adobe, and/or Access.Skilled and comfortable in conducting presentation/public speaking.Training in Lifestyle Balance curricula and other evidence-based prevention curriculum for at-risk youth and/or adults.Completion of and above-satisfactory scores on all job interviews, demonstrating to the satisfaction of the interviewees and TCRHCC that the applicant can perform the essential functions of the job.Successful completion of and positive results from all background and reference checks, including positive employment references from authorized representatives of past and current employers.Submission of all required employment-related documents, applications, resumes, references, and other required information free of false, misleading, or incomplete information.MENTAL & PHYSICAL EFFORT PHYSICAL: Must be able to exercise physical exertion when overseeing activities, including all types of repetitive bending, standing, floor exercises, and lifting up to 50 pounds. Requires normal range of hearing and vision. Travels and transports equipment/supplies to various communities and conducts program activities in rural communities within the Tuba City Service Area.
MENTAL: Must be able to prioritize, use good judgment, and be able to coordinate a variety of projects while being frequently interrupted. Must have a non-judgmental attitude and be comfortable working with low-resource individuals/communities.
Environmental: Will occasionally be exposed to dust, fumes and gases, infectious diseases, chemical agents, extremes in temperatures or humidity, hazardous or moving equipment, and unprotected heights and frequently exposed to loud noises.

ESSENTIAL FUNCTIONS Provide health coaching and support to Take C.H.A.R.G.E. Program and Lifestyle Balance Program participants through the delivery of evidence-based curricula in individual or group settings.Incorporate and integrate culturally appropriate concepts, modifications and activities when delivering health promotion/diabetes prevention education and services.Administers initial and follow-up health assessment to include and may not be limited to the following: blood pressure, pulse readings at rest, during and post exercise; FitnessGram administration; aerobic capacity testing, body composition measurements, and tests for muscular strength, endurance and flexibility.Assist with referrals and scheduling coaching sessions with all patients/participants of Take C.H.A.R.G.E. and Diabetes Prevention Program clinics and those referred from other clinics/programs of TCHRCC.Assist in reminder calls and follow-up coaching communication as needed to support participant recruitment, participation, and retention.Assist participants in setting personal health (health-related fitness, weight loss, etc.) goals and motivate them to adopt healthy behaviors.Orient and instruct individuals in proper use of exercise equipment and techniques in exercise.Identifies and utilizes appropriate resources for the participants.Assist with Take C.H.A.R.G.E. Program and Lifestyle Balance Program registry for grant and health outcome reports.Applies knowledge of behavior change theories and models to meet participant health goals.Works with care team (medical providers, health coach/es, etc.) on delivery of intervention plan for participants in the Take C.H.A.R.G.E. Program and Lifestyle Balance Program.Attend meetings as it relates to the grant or secondary prevention programs.Utilize appropriate media/marketing techniques in delivery of programs to increase program visibility.Deliver effective communication toward supervisor, program staff, program partnerships, hospital staff, and community members.Foster partnerships within TCRHCC as well as with outside organizations.Provides outcome reports and analysis for presentation; includes mandatory reporting.Understands and adheres to established compliance, safety, and quality assurance standards.Assists Community Connectors by providing distribution of food, water and other necessary essentials to community members who are in isolation or quarantine.Assists the Contract Tracing team by performing contact tracing activities for patients testing positive for COVID-19, and conducting negative results calling.Assists the Screening/Greeters at various entry points to ensure appropriate screening for all patients and visitors entering the TCRHCC campus.Ensure proper PPE is worn at all times while on duty.Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly.Complete task training for all routine cleaning and decontamination processes for all surfaces contaminated by a communicable disease.Performs other duties as assigned.
