TX Medicaid Cervical Cancer Screenings (Pap Tests)a
Health educators will assist with outreach to members and assisting with scheduling for their pap screening or well woman exam and other needed preventive screenings including but not limited to mammogram, annual wellness visit, A1c screening, blood pressure check.2
Medicare Statin Medication Referral Outreacha
Health educators will assist with outreach to providers advising them of members on their panel who may need to add a statin medication to their medication plan and scheduling an appointment with members to complete referral• What is the desired work hours (i.e
8am – 5pm) 8am-5pm with options for OT/alternate hours if needed (rare)• What additional equipment besides a laptop, keyboard, mouse and headset will be required for this candidate to be successful in this role?
(see below)• Dual Monitors• Keyboard• Mouse• HeadsetSummary: Develop, implement and evaluate health education programs for members within the network and within the community.Provide oversight and assistance to providers in meeting health education contract requirements
Additionally functions in a lead role over health educators and make administrative decisions as delegated.Essential Functions: Act as a resource to the health educators in the resolution of issues that may arise during the implementation of any intervention protocol
Make administrative decisions in the absence of the director or manager.Design, implement, and evaluate health education interventions to meet the needs of the targeted population based on priorities established by the department and current contracts.Conduct population specific, geographic specific and member specific needs assessments as required and as requested by contract.Act as a lead in the development of internal resources in collaboration with the Resource Specialist as identified within the annual work plan
Identify and evaluate health education services within the community
Use this information to extend resources to members by assisting with the development of referral agreements.Function as a liaison to internal departments, community-based organizations, social service agencies, and public health departments as assigned to ensure that resource information is current and available
Provide input on the design and functionality of all utilized databases.Attend work group meetings for maintenance and coordination issues
Coordinate the health education services assessment of IPAs and medical groups as required and collaborate with QI on oversight
Perform other duties as assigned including the submission of all required monthly reports.Knowledge/Skills/Abilities: Excellent verbal and written communication skills Ability to abide by policies Maintain regular attendance based on agreed-upon schedule Maintain confidentiality and comply with Health Insurance Portability and Accountability Act (HIPAA) Ability to establish and maintain positive and effective work relationships with coworkers, clients, members, providers and customersRequired Education: Masters degree required or equivalent experience or certification of specialization in patient education (such as CDE).Required Experience: 0-2 years of experience working with culturally diverse and low-income populations within a managed care setting
Supervisory experience required.
