Job Summary
The Registered Nurse provides intermittent skilled nursing services and directs, supervises, and instructs hospice aide staff in personal patient care.
Responsibilities include:
Assess and evaluate patient needs/problems, identifying mutually agreed-upon goals for the care plan with patients
Report patient status, updates, and discharge planning process (if applicable) to clinical leadership, attending physician, and hospice physician
Perform annual on-site visits to locations where patients receive care to observe and assess each aide
Attend staff meetings and educational in-services as needed
Ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Requirements
A current, unencumbered State Professional Nurse License
Valid driver's license and good driving record
Ability to work in a field setting and make sound nursing judgments
Experience with Microsoft Office
