Home Care Client Coordinator

Helping Hand Nurse is a Home Health Care agency servicing the greater Philadelphia Area and surrounding counties. We are currently looking for a Full Time Patient Care Coordinator.
The Coordinator will be responsible for:
Coordinating patient care with field staff, doctors, and organizationsConducting orientation/training for new caregiversEnsure compliance with caregiver clock-in and clock-out requirementsRequesting authorizationsComfortable visiting patients in their homes when needed for care managementDeveloping relationships with different Service CoordinatorsIncreasing satisfaction and caregiver/patient retentionCreating patient schedules and assigning staff to appropriate schedules promptly Helping Hand Nurse offers unique benefits that includes:

At least one year of home care experience is preferred but training is providedHave the ability to present a professional appearance and demeanorBe patient and congenial on the phoneStrong work ethicDemonstrate excellent oral and written communication skills and the ability to listen effectivelyHave the ability to work independently, maintain confidentiality of information and meet deadlinesDemonstrate effective interpersonal skills as well as sound judgment and good decision-making skillsBilingual (Spanish) preferred but not required Job Type: Full-time
