Overview Now offering $5,000 Sign On Bonus The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals.
5 x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends Great Healthcare benefits, and education reimbursement of $7,000 annually Additional compensation for visits over your caseload This position is on site at St. Francis Hospital in Roslyn, NY Job Details Our Home Care Transition Nurses: Establish and maintain professional relationships with key physician groups and referral sources.
Act as Liaison to referral sources on behalf of Catholic Home Care.
Identify potential market growth opportunities and program expansion.
Serve as a Customer service representative and act in a professional and supportive manner at all times.
Requirements: Registered Nurse with current licensure in NYS 2 or more years of home care experience preferred Customer Service and Marketing skills required Salary Range USD $100,000.00 - USD $115,000.00 /Yr.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience.
The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
