The Home Health Community Liaison is responsible for direct customer support, procurement of home health signed physician orders and signed CMS plan of care to include other required documentation. Assist home health medical records and clinical team managers in preparing the physician orders/CMS plan of care for delivery to physician office. Maintain professional and ethical manner and collaborates with other members of the health care team to ensure quality patient care and excellent customer service. Operates company vehicles safely and adheres to the responsibilities outlined in the Motor Vehicle Policy and Vehicle Fleet Safety Policy.
Primary Responsibilities:
Prepare physician orders/485s/Face to Face (F2F) for physician offices
Assists with checking in the signed documents once returned from the physician office
Assists with follow-up on physician orders/plans of care not returned within 30 days
Prepare spreadsheet weekly of outstanding documentation with required information for direct supervisor, director of operation of home health and home health VP
Daily route logs must be accurate and fully filled out and turned in daily to direct supervisor
Daily vehicle inspection must be performed daily and recorded on route sheet
Responsible for setting up vehicle maintenance/oil changes and reporting any vehicle concerns to direct supervisor and Fleet director at Wellmed
Delivery and pick up of physician orders and plan of care's to be signed by physician twice weekly
Responsible for tracking missing documentation and following up to ensure timely receipt according to the applicable laws, federal, and state rules and regulations and Agency's policy.Assist physician office pertaining to accuracy of documentation being delivered and work with home health clinical team managers for any corrections needed on the documentation-needing signature by physician
Assist physicians with information in reference to electronic signature
Receives and addresses customer complaints by routing specific or complex questions to appropriate managers
Exhibits professionalism and is courteous with all customers and co-workers
Demonstrates knowledge and observance Notice of Privacy Practices
Participates in educational programs and all required in-service programs, as assigned by supervisor
Adheres to the time reporting policy of updating times in Global self-service in real time to include prior approval for overtime per UHG policy
Requires obtaining pre-approval from direct supervisor for overtime
Complies with all Homecare Dimensions, Inc. Agency's policies and procedures
Adheres to the attendance policy per UHG. Required to arrive to work per scheduled work time and wear business casual clothing
Acknowledges and understands that each driver is to operate company vehicles safely and adhere to the responsibilities outlined in the Motor Vehicle Policy
Fuel cards remain in car assigned
Basic knowledge with current home health EMR.Performs additional duties as assigned by manager
Coordinate and perform minimum of four Inservice's yearly with physician clinics/hospitals

High School Diploma/GED (or higher)
2+ years of customer service experience
Intermediate level of proficiency with MS programs including Excel and Word
Possess and maintain at all times a valid Texas Drivers' License and personal auto insurance coverage in accordance with organization requirements
Preferred Qualifications:
Experience in home health or clinic setting preferred
Public speaking experience preferred
Soft Skills:
Must be able to lift at least 25 lbs independently
Must be able to sit or stand for extended periods of time
Must be able to effectively read and follow written and verbal instructions in English
Proven oral communication skills including telephone etiquette
