Facilitates, and coordinates delivery of agency services by ensuring accurate demographic information and appropriate physician orders.
Assumes responsibility and accountability for facilitating, communicating, and collaborating with both the healthcare team, and the patient/family to ensure an outstanding customer experience.
Promotes the optimal health, well-being and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act
Qualifications:
In addition to bringing humankindness to the workplace each day, qualified candidates will need the following: High School Diploma/G.E.D (Required) Graduate of an accredited Licensed Practical Nursing program (Required)Two years of recent LPN experienceOne year of home care, hospice or home infusion experience preferredPrevious experience in a discharge planning, preferredRequires knowledge in nursing, evidence-based practice methods, and person-centered care conceptsCurrent state LPN license for applicable state of employment or current LPN license from a participating state in National Compact LicensureCurrent Cardiopulmonary Resuscitation (CPR) requiredDrivers LicenseLPN IV certification preferredCurrent Basic Life Support (BLS) certification from American Heart Association, preferred Medium Work - exert/lift up to 50 lbs.

The hours are 8:00am to 5:00pm.
We are looking for at least 2 years of LPN experience.

$20.66 - $34.68 / hour
