We are seeking a Home Health Physical Therapist to join our team in Berwyn, Oak Lawn, Oak Park, LaGrange, and Cicero! You will plan physical rehabilitative programs to improve patient quality of life.
Responsibilities: Develop and implement physical therapy programs that focus on rehabilitationWork directly with patients to achieve maximum physical recoveryEvaluate effects of therapy treatment and communicate patients' progressRecord and document patient care servicesCollaborate with other team personnel to achieve well-rounded careWork professionally with all levels of staff membersEnsure compliance in all aspects of patient care and medical documentationSee patients as directed by Vice President of Clinical OperationsQualifications: Previous experience in home health careAbility to communicate effectivelyAbility to handle physical workloadAbility to build rapport with patientsExcellent written and verbal communication skillsStrong leadership qualitiesDoctorate in Physical Therapy
