Home Health Rn Care Coordinator

Reporting to the Nurse Manager, the Home Health RN works inconjunction with the patient's physical andinterdisciplinary team to coordinate and provide skilled nursing care topatients in their place of residence.
The RN performs professional rolefunctions in accordance with scope of practice and professionalstandards, including demonstrating the knowledge and ability to use and/or leadthe scope of practice, professional standards and accreditation / regulatorystandards, scope of practice for LVN and all other nursing personnel e.g.unlicensed assistive personnel.
The RN demonstrates the ability to assesstheir own professional role performance, current knowledge of theory andprinciples related to discipline specific practice, and the ability to use apopulation based approach to determine patient needs.



Current CA State licensure as a Registered Nurse required Current CPR certification required.
(American Heart Association only) One year experience as a professional nurse required Valid CA driver's license and an automobile, with current registration and insurance for field work required
