PPCCC is a trusted provider of high-quality, affordable reproductive health care along California's Central Coast. Each year, PPCCC provides 35,000 people on the central coast with health care services; including sexually transmitted infection testing and treatment, birth control, breast and cervical cancer screenings, vasectomies, and safe and legal abortion care at our six health center locations. PPCCC also provides comprehensive sexuality education programs to help adults and teens make healthy decisions, prevent unintended pregnancies, and avoid sexually transmitted infection.
JOB SUMMARY: The Home Health RN Manager is responsible for developing, introducing, managing, and overseeing the affiliate's postpartum home health program in collaboration with the Clinical Services Department. This position will also provide direct patient care in-person via home health visits, telehealth, and in-clinic visits. As a member of the clinical services team, the Home Health RN Manager will work directly with the licensed and non-licensed medical staff to assess medical, nutritional, behavioral, and family planning needs and provide care coordination and referrals to families in homes or at other locations through a team-based, whole family care approach.
ESSENTIAL FUNCTIONS: · Develop standards of care, protocols and policies for postpartum home health care provided within our affiliate for PPCCC's Medical Standards and Guidelines. · Partner with outside postpartum programs and related organizations to facilitate timely referrals and support networks. · Develop an efficient process specific to PPCCC for identifying and home-visiting patients in need of postpartum health services · Develop and maintain a routine system of ongoing training, supervision, and quality assurance related to postpartum health for unlicensed and licensed home health providers. · Establish and maintain appropriate quality standards for patient care in postpartum health. Work in collaboration with the Clinical Quality and Risk Manager to develop audits to track compliance. · Hire and onboard additional postpartum health providers including registered nurses and doulas as the program expands. · Provide direct patient care including postpartum health screenings for perinatal mood or anxiety disorder (PMAD), birthing parent and newborn physical assessments, and lactation assessment and support for breast/chestfeeding families, as well as referrals to community partners. · Assess housing, environmental, and safety needs, provide assistance as needed, and make appropriate referrals as needed. · Serve as a clinical resource to licensed and non-licensed staff with respect to postpartum health support and resources. · Supervise postpartum home health registered nurses, doulas, interns, and students as assigned. · Provide consultation and training for staff to ensure compliance with PPCCC standards of care and all legal/ethical obligations pertaining to postpartum home health. · Perform other duties as assigned by the Chief Medical Officer or designee.

· Public Health Nurse Certification from the California Board of Registered Nursing
· Current BLS/CPR licensure · Certification as an International Board-Certified Lactation Consultant (IBCLC) preferred · Experience providing services via home health visitation is required. · At least 3 years experience in postpartum and infant feeding direct patient care. · Prior experience in sexual and reproductive health care preferred. · Prior work with diverse patient populations preferred. · English/Spanish bilingual skills desirable.

Each position has a salary based on market value and the organization's pay levels. The starting salary for this position is $ 120.385/year with an opportunity for an increase after six months of employment. As part of our commitment to pay equity, PPCCC does not negotiate salaries.
Pay Differential: $1.00/hour bilingual Spanish/English differential pay is offered upon passing the language assessment test.
