Home Health Visiting Nurse - Skilled Nursing Services

Job Summary HarmonyCares is a leading home-based primary care practice seeking a skilled Home Health Visiting Nurse to provide high-quality, coordinated healthcare services to patients in their homes.
Key Responsibilities:
Assess and evaluate patient needs and develop individualized care plans to meet those needs
Provide intermittent skilled nursing services, including assessment, evaluation, procedures, teaching, and training activities
Communicate patient progress with other disciplines and direct, supervise, and instruct non-professional home health aide staff in personal care
Report patient status and need for other disciplines to agency Clinical Supervisor and referring physician
Requirements:
Current unencumbered State professional Nurse License
Ability to make sound professional clinical judgment and assess and document patient needs
Proficient clinical skills and excellent verbal and written communication skills
Proficiency in personal computer use, including email, clinical, word processing, spreadsheet, and presentation software
