Provides direction and supervision for the interdisciplinary team members to promote Trinity Health values in proactive patient care management activities.
Essential Functions The duties and responsibilities described are not a comprehensive list and additional tasks may be assigned from time to time, as the scope of the job may change as necessitated by business demands Hospice Admissions manager establishes, re-evaluates, promotes, and execute the admissions process from referral to patient admission.
Oversees the Admission Team under the guidance of the associate clinical director, to ensure that the referral/admission process is performed in a smooth and coordinated manner.
Serves as a resource to the Admissions team and works closely with hospice medical director.
Responsible for timely referral visits and admissions in collaboration with scheduler; that referral visits are assigned appropriately taking into consideration staff, geographic location and needs of the patients; ensures timely follow up on all pending patients.
Transfer of patients from Admissions to Patient Care occurs in a smooth and coordinated manner.
Responsible for preparing end of the day report for on-call staff, including assignment of planned after hours admissions.
Admission manager reports off at end of the day huddle regarding any patients admitted, NTUCs, pending patients, and planned afterhours admissions.
Admissions manager, in collaboration with admissions coordinator, manages pending referrals list.
Including the planning, documenting, and executing pending list follow up plan according to patient's specific needs.
Provides direct supervision of the Admission RNs.
Ensures adequate staffing levels; monitors maintenance of schedules.
Ensures proper orientation of employees.
Oversees utilization of GIP contract bed admissions, manages approval process for non-formulary treatments/medications all pending referrals.
Oversees admissions for all hospice patients, ensuring the level of care is appropriate under the direction of the hospice medical director, in collaboration with the attending physician, in accordance with all Federal and state regulations Oversees admissions to CbPC in accordance with all policies and protocols, where services are available.
Monitors departmental productivity and ensures the productivity of the Admission Nurses and liaisons is consistent with the standards and goals of the department's conversion rate; including timely conversion of pending and follow-up referrals to admission or the organization's continuum of care.
Assists the department director in monitoring and reporting quality improvement and corporate compliance initiatives.
Works to ensure compliance with documentation.
Assures that all initial assessments are conducted by qualified team members within required time frames, including documentation completion, submission and/or synchronization.
Participates in the selection and evaluation of all direct patient/family care team members Educates, participates, and promotes bedside clinical skills by reinforcing methodical practice for determining hospice eligibility, symptom management, level of care determination, and development of appropriate initial plan of care for hospice admissions.
Provides external education/in-services to community partners as necessary.
Supports provider, facility, and system hospital relationships through establishing partnership guidelines as necessary, to assist with seamless transitions of care.
Collaborates with Site Leadership to support staff education efforts Participates in the orientation and ongoing education of staff and volunteers.
Responsible for preparation and execution of regularly scheduled admission nurse and liaison meetings in collaboration with site leadership.
Admission Meeting Preparation: Prepares patient / care quality data analysis to provide timely staff support during huddles and gives feedback as necessary during and/or after meeting Facilitates interdisciplinary meetings, as needed.
Prepares and follows up on identified patient care concerns Ongoing IDG Support: Utilizes proactive management strategies to assist staff, patients and their families, which may include:
Triages incoming referral calls during business hours
Provides caller support and assistance
Performs quality calls
Documents all above per protocol
Will assume responsibility for direct patient care and administrator on call duties as required.
Quality Investigates (root cause analysis) for any referral and/or admission concerns and follows up with staff / process issues in collaboration with site Leadership as needed, and documents outcomes per protocol.
Participates in or leads quality improvement activities, including data collection and aggregation as able Participate in ongoing quality assurance activities related to patient and family services, including Quality Assurance & Performance Improvement (QAPI) studies and chart audits to ensure the quality of patient/family services.
Communications Reinforces and supports all internal / external communications related to:
Interdepartmental information
Administrative process changes / messaging
Participates in the development, implementation and evaluation of policies and procedures for patient/family services.
Ensures all interdepartmental communication and exchange of data is accurate, timely and effective.
