Job Type Full-timeDescriptionThe Hospice and Palliative Clinical Manager coordinates daily operational management of the hospice and palliative agency patient care services.
Responsibilities include development of the staff under a case management philosophy, maintaining quality patient care, promoting productivity and teamwork of staff.
ESSENTIAL DUTIES AND RESPONSIBILITIESAssists in the development, implementation, and revision of clinical standards and policies and procedures as needed.
Oversees daily operations of RN, and LPNs, provides orientation, and holds all direct reports accountable for performance, conducting performance reviews/coaching/disciplinary meetings with staff as necessary.Completes weekend on-call per rotation and is responsible for assisting with call coverage as needed.
Participates with the physician, referral sources, and professional staff in decisions regarding patient eligibility and suitability for the hospice program.Provides ongoing nursing education to enhance the quality of nursing care.Assesses nursing team on an ongoing basis to ascertain their understanding and compliance with policies and procedures.Assists in follow through of patient transfers with facilities and hospitals as needed.
Back up to Clinical Coordinator to manage the hospice task hotlist on a daily basis.
Oversee care delivery and patient outcomes to ensure that care meets the patients' needs.Serves as a resource to staff regarding appropriateness of hospice coverage for medications, equipment, treatments, procedures, labs, etc.
Coordinates and ensures competencies for new employee orientation and Adoray's perfect visit implementation for hospice and palliative patient care staff.Responsible for effective utilization of staff including assignment and reassignment of duties.Coordinates assignments and evaluates appropriateness of client care.Take call and patient visits as needed.Monitors and intermittently audits clinical documentation for consistency, timeliness of completeness and accuracy and assures the development of, implementation and updating of the individualized plan of care.Assists in admission and discharge chart audits as needed.Knowledge of Matrixcare "back office" and ability to assist with tasks such as: Order processing Scheduling Referral input Various reports Assesses and arranges for staff in-service needs and participates in in-service programs.Conducts supervisory visits with all hospice and palliative field staff annually and as needed.Makes staffing assignments or re-assignments and otherwise ensure adequate coverage.Provides instruction or guidance to staff concerning procedures for providing nursing care.Holds CIT meetings with hospice IDT teamPreps and leads palliative IDT meeting Helps complete/attend hospice clinical meetings.Provides problem resolution for clinicians, patients, families, and physicians daily as backup to the Hospice Clinical Coordinator.When problems are identified related to patient care, implements solutions in consultation with Director of Quality and Clinical Services and/or Executive DirectorProvides instruction or guidance to staff concerning procedures for providing nursing care.
Applies specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient's eligibility.
Implements the individualized plan of care and recommend revisions to the plan as necessary including managing discomfort and providing symptom relief and specialized nursing skills related to palliative and end-of-life care.
Continually works to anticipate hospice patient's needs regarding symptom management, thus minimizing the likelihood of a patient care crisis.
Assists in follow through of quality initiatives with QAPI programAssists in Emergency Preparedness tasks.Backup for Enrollment team as related to Hospice and Palliative referrals and admission.PRN visits to back up staff Other duties as assigned.
