Come join the fastest-growing hospice care in the Midwest.Schedule: 3-12 hour shifts per month minimum requirementWhat We offer:Great culture and team atmosphere401(k) retirement plan with a generous company matchMileage reimbursementEmployee Referral Program benefit and salary increaseMerit IncreasesNight Shift Differential Employee Discount ProgramsWhat You'll Do:Provide direct patient care as defined in State Nurse Practice Act Implement current nursing practice following a comprehensive assessment and the plan of care Provide accurate and timely documentation of patient services to reflect the plan of care Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
Provide appropriate pain/symptom management.
Evaluates the patient's response to treatments/medications Qualifications:Graduate of an accredited practical nurse or a vocational nursing program Three years of nursing experience.
