Hospice Rn - Case Manager

Regional Hospice is looking for a FULL TIME RN Case Manager for our Home Hospice patients.We are looking for full time 40 hours, day shift. The Case Manager works with interdisciplinary team and the patient's primary care physician to provide quality pain and symptom management, as well as support to the patient and family.Qualifications* Registered Nurse with a current license in the State of Connecticut required.* Bachelor of Science in Nursing degree required.* Two (2) years' experience as a staff nurse in a hospital and at least one (1) year experience in hospice or home health care agency required.* Advanced credentials in hospice care preferred (CHPN).* Transportation required, including proof of auto insurance and a clear driving history.* Excellent verbal, written and computer skills – ability to communicate effectively with patients, families, physicians, interdisciplinary team members and others.* Fully Vaccinated for Covid-19The RN Case Manager must possess good communication and organizational skills and be able to work in a variety of environments.
