Position Summary: The Hospice Social Worker works as a member of the interdisciplinary team working with terminally ill patients and their families. The social worker's services are provided in accordance with Medicare guidelines and the patient's plan of care, under the direction of a physician. Typical tasks include consultations, admissions, initial and ongoing assessments, care planning, care coordination, end of life education, supportive counseling, resource/referral support, discharge planning.
