Job Description - Hospitalist, Full Time Days, Monadnock (2301265) Hospitalists in the Hospital Medicine Department are clinicians who provide clinical care for patients admitted to Catholic Medical Center and provide consultative internal medicine services to surgical and specialty colleagues.
DUTIES AND RESPONSIBILITIES:Hospitalists will engage in direct patient care activities and medical/clinical practice as the principal component of his/her overall professional responsibility within the Department. Patient care activities will include, but are not limited to:
Admission, assessment, evaluation, diagnostic determination, ongoing treatment and clinical management, necessary referral for consultation, care planning and modification, medications/therapeutic orders, transfer authorization, risk assessment, and, when appropriate, determination of readiness and planning for discharge for inpatients at Catholic Medical Center.Direct contact and consultation with patients and, when permitted, with family members, friends, advocates, and others to ensure effective communication of patient status and care plans.Appropriate and ongoing communications with ambulatory primary care physicians who routinely manage the care of patients currently in the hospital.Day-to-day patient contact/rounding/care management that actively encourages and includes input from all members of the patient care team.Accurate and timely completion of all clinical and professional records and documentation to assure optimal patient care and compliance with local, state, and federal regulatory, licensure, and accreditation requirements.Work to create a positive work culture within cross-departmental teams and align this work with other Hospital Medicine initiatives.Perform other related duties incidental to the work described herein.Education:Work requires an MD, DO, and completion of advanced graduate medical education in Internal Medicine or Family Medicine with board eligibility or board certification in internal or family medicine.
Experience:Internal Medicine: Successful completion of training experiences as mandated by the ABIM for board eligibility is required. Clinical experience in an inpatient healthcare environment caring for adult patients. Current ACLS certification.
Family Practice: Successful completion of training experiences as mandated by the ACGME for Family Medicine residency program. Board certified with two or more years of experience as a Hospital medicine physician with ICU experience. Current ACLS certification.
Family Practice: Successful completion of training experiences as mandated by the ACGME for Family Medicine residency program. Board certified and completion of one-year fellowship in Hospital Medicine with ICU experience. Current ACLS certification.
Degrees, Licensure, and/or Certification:Physician will maintain a current unrestricted license to practice medicine in the State of New Hampshire, federal and state prescriptive authority, and will obtain and maintain board certification by the American Board of Internal Medicine/American Board of General Medicine in accordance with the Medical Staff Bylaws of Catholic Medical Center.
Knowledge, Skills, and Abilities:Technical: Clinical training and skills as mandated by the American Board of Internal Medicine for General Internal Medicine physicians.Clinical training and skills as mandated by the American Board of General Medicine.Ability to work with electronic medical records.Non-Technical: Ability to work with others in a cooperative, collaborative manner and within a cross-cultural situation.Strong written and oral communication skills.
