The Housing Navigator functions as a facilitator between the Street Medicine, Care Management departments and Outpatient clinics with regard to the coordination of homelessness resources.
Under the direction of Social Services and Street Medicine leadership, the Housing Navigator assists MLK staff with linking patients experiencing homelessness with homeless resources throughout the community, and is responsible for building relationships with community-based organizations that will best serve Martin Luther King Jr. Community Hospitals' patients and their families.
ESSENTIAL DUTIES AND RESPONSIBILITIES Outreach Coordination: Develops relationships with community-based organizations, who specifically serve the homeless community, including city, county and other hospital facilities.
Is a point of contact for the Street Medicine and Social Work Emergency Room Teams for homeless programs and/or enrollment in healthcare provider programs (shelters, recuperative care programs, recovery programs, with an emphasis on the Department of Health Care Services Enhanced Care Management and Housing Navigation, Programs, etc.).
Keeps the department current on changes in program requirements.
Provides ongoing education and resources for the homeless community to Street Medicine and Care Management staff on new programs/facilities serving the homeless community.
Participates as a successful team associate, specifically supporting data collection and health outcomes reporting.
Follows up with homeless patients who are at high risk for re admission, in an effort to encourage the use of resources provided to them during their hospital stay or ED visit.
Provide outreach in the field if needed to follow up with enrolled members of HHSS program.
Manage a caseload of HHSS enrolled patients, max 30.
Works with the hospital Interdisciplinary teams as requested by leadership.
Performs other duties as assigned.
POSITION REQUIREMENTS A.
Education Bachelor's Degree in Social Work, Public Health, or related degree required.
B. Qualifications/Experience One (1) year experience in a community outreach role or working with the homeless community.
One (1) year experience in a healthcare facility preferred.
Highly organized and well developed oral and written communication, problem-solving, and decision-making skills.
C. Special Skills/Knowledge Current Basic Life Support (BCLS) for Health Care Providers from the American Heart Association
