Responsibilities:Perform consultations based on the results of the biometric screenings performed:Blood pressure, BMI, and height and weight measurementsCholesterol and Glucose screeningsOsteoporosis screeningsPerform the following screenings during the consultation:Mild Cognitive Impairment screeningPHQ-2 screeningMedication adherence discussionShare a list of local providers available on iPadQualifications:Certified/Licensed Nurse Practitioner, Physician Assistant or Registered NurseStrong interpersonal and communication skillsAbility to work in a fast-paced environment with attention to detailFamiliarity with medical equipment and technology
