Hybrid Rn Case Manager - Registered Nurse - Chha

Compass Healthcare Consulting & Placement is seeking an experienced RN Case Manager, Registered Nurse, Coordinator of Care to work for our client a large Certified Home Healthcare agency with offices in Mount Vernon, New Hyde Park, Staten Island and Brooklyn, NY.
Position is in-office with a great clinical team!
Work 3 days per week from 1 location and 2 days from home!
Will train RNs with CHHA, LHCSA or MLTC Case Management experience!




The Registered Nurse, RN Case Manager works in office collaborating with field staff to oversee and ensure clinical documentation adheres to NYS DOH and CMS guidelines.
Provides clinical support and guidance to field staff as needed.
Collaborates with Providers, Clients, Caregivers, Vendors and ensures safe/effective Care Plans are established.
Promotes Quality and Service Excellence at all times.




Registered Nurse, RN Case Manager Qualifications:




Registered Nurse in New York State Required


Recent experience working for a Certified Home Health Agency in New York State


Knowledgeable regarding Medicare Guidelines, COPs for CHHA.


Ability to work in a Team setting.


Previous Field or Office - Coordination of Care in a CHHA setting experience Required.


Clinical Manager or Patient Service Manager experience a PLUS!


Strong written and oral communication skills needed.


Working knowledge of Electronic Medical Record/McKesson/Home Care Home Base a plus!
