Mental Health Cooperative, Inc. (MHC) was formed in 1993 to serve individuals with severe and persistent mental illness. Since then, we have expanded our services to children and adolescents with severe emotional disorders across Middle and East Tennessee.
Our sole purpose is to support and treat those challenged with serious mental illness and poverty. Job Title: Intensive Behavioral Health Care Manager, PACT
Schedule: Mon-Fri, 8a-4:30p
Salary: $43,500 (base pay) with additional incentive for language skills and behavioral health experience. Mileage reimbursement provided.
JOB SUMMARY: This member of the multidisciplinary team has responsibility to carry out rehabilitation and support functions and assist in treatment, substance abuse services, education, support and consultation to families; and crisis intervention under the clinical supervision of staff with professional degrees.
RESPONSIBILITIES: Provide case management for an assigned group of consumers including coordination and monitoring the activities of the individual treatment team.Responsible for developing, writing, implementing, evaluating and revising overall treatment goals, plans and concurrent reviews in conjunction with Individual Treatment Team (ITT).Provide individual supportive therapy and symptom management.Educate and support consumers' families.Advocate for the rights and preference of consumers.Conduct comprehensive assessment of psychiatric history, mental status, diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living; and family structure and relationships.Act as liaison and consult with community agencies and families to maintain coordination in the treatment process.Perform shift management according to established policies and procedures.Provide on-call crisis intervention according to established policies and procedures.Document consumer progress to maintain a permanent record of client activity according to established policies and procedures.Provide support services to the treatment and rehabilitation of consumers.Participate in daily staff organizational meetings and treatment planning review meeting and monthly all staff meetings.Provide on-going assessment of consumers' mental illnesses symptoms and consumers' response to treatment.Provide symptom education to enable consumers to identify their mental illness symptoms.Provide direct clinical services to consumers on an individual group and family basis in the office and in community settings to teach behavioral symptom-management techniques to alleviate and manage symptoms not reduced with medication and to promote personal growth and development by assisting clients to adapt and cope with internal and external stresses.Provide multiple-stage individual and group treatment in the office and in community settings to develop a trusting relationship with clients so that they may more freely discuss substance use and its effect on mental and physical health and daily functioning.May participate in setting treatment goals and plans with clients to reduce or abstain from substance and to learn replacement behaviors.Coordinate with outside inpatient services to detoxify clients and establish linkage to outpatient treatment, self-help programs, outpatient services and residential facilities.Participate in the provision of rehabilitation services.Provide individual vocation-supportive counseling to enable consumers to identify vocational strengths and problems, establish vocational or career goals and plans to reach them, and recognize and target symptoms of mental illness that interfere with work.Plan and provide work-related supportive services, such as assistance with grooming and personal hygiene, securing of appropriate clothing, wake-up calls and transportation.Provide benefits counseling.Provide ongoing assessment, problem solving, side-by-side services, skill training, supervision (e.g., prompts, assignments, monitoring, encouragement) and environmental adaptations to assist clients with activities of daily living.Assist and support consumers to carry out personal hygiene and grooming tasks.Provide nutrition education, meal planning, grocery shopping and food preparation.Assist consumers to find and maintain a safe and affordable place to live – apartment hunting, finding a roommate, landlord negotiations, cleaning, furnishing and decorating, and procuring necessities.Assist and support consumers to perform household activities, including house cleaning and laundry.Ensure that consumers have adequate financial support.Teach money-management skills and assist clients in accessing financial services.Help consumers to access reliable transportation.Assist and support consumers to have and effectively use a personal physician and dentist.Provide individual supportive therapy (e.g., problem solving, role-playing, modeling and support), social-skill development, and assertiveness training to increase consumer social and interpersonal activities in community settings.Plan, structure and prompt social and leisure-time activities on evenings, weekends and holidays.Provide side-by-side support and coaching to help consumers socialize.Organize and lead individual and group social and recreational activities to structure consumers' time, increase social experiences, and provide opportunities to practice social skills and receive feedback and support.Provide practical help and supports, advocacy, coordination, side-by-side individualized support, problem solving, direct assistance, training, and supervision to help consumers obtain the necessities of daily living including medical and dental health care; About MHCMHC was founded in Nashville, Tennessee, in response to the need for assertive community-based treatment for individuals who have required frequent inpatient psychiatric hospitalization. Since 1993, MHC has been passionately helping adults with severe mental illness to live quality lives in the community with our state-of-the-art treatment and support. In 1999, MHC recognized the need to support the whole family and expanded services to include children and youth.
We established our non-profit agency by merging three community organizations: Urban Case Management of Meharry Community Health Center, Continuous Treatment Team of Council of Community Services, and Mobile Crisis Team of the Crisis Intervention Center.
