Internal Medicine position in CT_x000D_Located in Southbury_x000D_22m to New Haven; 41m to Hartford_x000D_Full-time; permanent_x000D x000D_Job Summary:_x000D_Group is actively seeking a board-certified physician to join our group, providing high quality primary care in an office-based setting. _x000D x000D_Job Responsibilities:_x000D_Examine, obtain medical history and treat acute and chronic illnesses._x000D_Order, perform and interpret diagnostic tests._x000D_Create treatment plans, and involve patients and family members in those plans._x000D_Initiate appropriate specialty referrals._x000D_Collaborate on improvement in care delivery and service with management, other physicians and staff._x000D_Work with care team to close care gaps and improve quality measures._x000D_Participate in Patient-Centered Medical Home (PCMH) and Accountable Care Organization (ACO) initiatives. _x000D_Work with practice management to ensure compliance with billing and coding requirements._x000D_Provide on call coverage within the primary care group._x000D x000D_Education/Licensing Requirements:_x000D_Graduation from accredited medical school (MD or DO)._x000D_Graduation from accredited Internal Medicine or Family Medicine residency program._x000D_Valid and unrestricted Connecticut medical license or ability to obtain._x000D_Valid DEA registration and Controlled Substance Certificate._x000D_Board certified in Internal Medicine or Family Medicine._x000D x000D_Compensation & Benefits: _x000D_Competitive salary plus incentive compensation based on productivity and quality measures.
