People First Team Collaboration: Become part of a cohesive and long-standing team of 50 physicians.
Work alongside a diverse set of prior clinical experienced professionals including hospital-based, outpatient, or post-acute like nursing homes.
Qualifications: Applicants must have an MD or DO and be board-certified in Internal Medicine (IM) or Family Medicine (FM) with at least three years of hospital experience.
Candidates should hold a minimum Compact/IMLCC qualification and possess three or more active state medical licenses, preferably in states such as CA, NY, FL, MA, CT, MD, VA, NJ, PA, NC, and SC.
Excellent interpersonal and verbal communication skills are required.
Prior experience in telemedicine is preferred.
Experience in post-acute or skilled nursing facility (SNF) care is also welcomed.
Preference will be given to candidates with a California medical license and a high number of additional state licenses, including Compact licensure.
Practice with your Heart Flexible Scheduling: This position is fully remote, allowing you to work in a virtual environment.
It is a full-time opportunity, requiring a minimum of 120 hours monthly.
The schedule is released in advance to help you plan other commitments, with team members providing coverage for each other during absences.
Key Responsibilities: Manage the complexities of caring for patients in post-acute environments.
Engage in cross-coverage cases via telemedicine to provide continuity of care.
Be available for after-hours shifts, including evenings, nights, weekends, and holidays.
Participate in monthly team meetings to discuss practice changes or updates.
Receive comprehensive training on technology protocols, workflows, and electronic medical records (EMR) systems.
Purpose-Driven Work Patient-Centered Care: Our providers are located across the country and can work from home while serving a much-needed population in post-acute facilities like nursing homes to prevent unnecessary ER visits and unnecessary hospitalizations.
Rewards and Benefits: Compensation: Salary range $136-165K annually pending interview process and experience.
