Two years of community health center primary care experienceWe have an immediate need for an Internal Medicine Physician with expertise to join our healthcare team in Seattle, WA. The ideal candidate will have a strong background in providing comprehensive medical care for patients, addressing their unique health needs. Job Overview Under the supervision of the Medical Director, the primary care physician will provide comprehensive medical care to the participants of the Program for All-Inclusive Care for the Elderly to aid in their ability to age in place. Responsibilities include initial and periodic assessments, follow-up care on chronic medical conditions, collaborating with an interdisciplinary team to formulate and execute plans of care, and managing all aspects of a patient's medical care in conjunction with the interdisciplinary team for care coordination. This position plays a key role in working with the Medical Director to aid in workflows and procedures to ensure that the highest quality of care is provided to the patients. The primary care physician should demonstrate compassion, patience, medical knowledge, and skills necessary to assess, plan, care for, and provide service to our most vulnerable population.
