Internal Medicine Physician North Hudson Community Action Corporation in West New York, NJ, seeks an Internal Medicine Physician.
The role involves:

Providing health maintenance for patients and ensuring Standard of Care within the Internal Medicine Group. Analyzing and interpreting data, formulating diagnoses and problem lists, and establishing management plans for common and complex adult health problems. Treating problems within the scope of competence and exercising judgment on issues requiring consultation, referral, or evaluation. Providing patient education in a linguistically appropriate manner regarding health matters. Managing common and complex chronic adult illnesses. Responsible for the overall clinical management of all Adult/Family medicine medical providers and monitoring on-site programs to meet the organization's goals, quality performance measures, regulatory compliance, training programs, and research initiatives. Precepting and collaborating with other staff members/disciplines to improve the patient experience. Participating in team meetings and activities of the PCMH (Primary Care Medical Home) directly or indirectly. Entering into Joint Collaboration Agreements with one or more NHCAC Advance Practice Nurses. Conducting peer and performance reviews as assigned and monitoring Site Quality Performance. Minimum Requirements: MD in Medicine and 3 years of Medical Residency Training. Active Medical License in the State of New Jersey. Board Certification in Internal Medicine. Current BCLS and DEA Certificate. Salary: $180,000 to $190,000.
