Keeping Families Together Clinician (Per Diem)

Keeping Families Together is a long-term program aimed to assist DCPP involved families challenged with homelessness/housing instability putting children at risk for out-of-home placement or delaying reunification. KFT utilizes a housing-first model to work with our families in obtaining stable, subsidized housing. The program focuses on the following to help families overcome the obstacles that prevent the family unity they desire:

Responsibilities: As a member of a multi-disciplinary treatment team provide direct care as delineatedProvide therapy as clinically indicated for individuals and family unitsEngage all consumers and their family members in treatment planning and all areas of service deliveryAssist consumers with ADLs through modeling, direct assistance and coachingProvide assessment of client's emotional well-being, clinical symptomology, behavioral changes and medication complianceEvaluate significance of SU history and current use if applicableProvide psycho-education for consumers and family members on mental illness, symptom management, medication compliance and life skillsMinimum Requirements: Master's Degree in Social Work, Mental Health or Human Services Related FieldLSW/LAC or LCSW/LPCExperience with entitlements and community system of careAdditional Benefits: Opportunities for training/education/Continuing Education CreditsOpportunities for Tuition Discounts from Participating Institutions
