GENERAL JOB DESCRIPTION
The residential Direct Support training and direct support to individuals with mental illness in their homes that includes preparing meals, home upkeep, medication monitoring, engaging in community activities and attending appointments.
QUALIFICATIONS Must be over the age of 18Valid Michigan driver's license, three (3) years of driving experience, and must have own vehicle at work, vehicle insurance and registrationFlexibility, knowledge of basic housekeeping skills DUTIES AND RESPONSIBILITIES Support participants with disabilities to reach their independent living goals outlined in their IPOS and provide transportation.Support participants in accessing recreational, cultural, and religious opportunities in the community.Flexibility to work varied shifts/locations based on the needs on the agencyAssist clients who have mobility, balance, strength and/or physical limitations with their ADL's.Provide transportation for residents to meet their needs including shopping, appointments and other community engagement activities/events.
Assist and support clients with household responsibilities and/or perform house chores when clients refuse or are unable to do so, e.g.
laundry, meal prep/clean upAccurately administer and document medications passed to participantsWrite daily progress notes outlining participant activities, interactions with others and incidents observed during every shift in DARTS.
Attendant all mandatory meetingsFollow all emergency procedures and participate in monthly drillsAct as a positive role model for clients and co-workersFollow the Adult Protection Code, Mental Health Code, Gesher Values, and the Commission on Accreditation of Rehabilitation Facilities (C.A.R.F.)
guidelinesComplete all assigned trainings and shadow in various homesComplete weekly medication reviews and monthly medication checks.
Assist with completing assessments, goal planning and IPOS development.Provide support and training to other staff including serving as a trainer for apprentices as assigned.Cross-trained to provide direct service support/coverage to residents in various homes/settings.Other duties assigned WORKING CONDITIONS

Physical Requirements:
Work involves the ability to work flexible hours and to travel to local community sites.
Work involves sitting for a minimum of 60% of work timeAbility to stoop and bend to file recordsAbility to type and read computer screen
Environmental Conditions:
Moderate noise (i.e., business office with computers, phone, and printers, light traffic).Ability to work in a confined area.Ability to sit at a computer terminal for an extended period.
